17-hydroxyprogesterone (17-OHP) is a steroid hormone, derived from cholesterol, and is 1 of the 3 analytes along with cortisol and androstenedione, that constitutes the best screening test for congenital adrenal hyperplasia (CAH). CAH is an inherited condition that results in decreased adrenal cortisol and aldosterone and increased male sex hormone (androgen) production, caused by either 11- or 21-hydroxylase deficiency. Analysis for 17-OHPG is also a useful test to evaluate females with hirsutism or infertility.
